I use my own modelcontract. And I have a cancellation policy.
“Model has travel expenses and it is very hard to book shoot last minute, Photographer have to pay cancellation fee and part of travel costs. There is a 50% cancellation fee if the shoot is cancelled 7 days prior to the scheduled shoot. There is 100% cancellation fee for cancellations within 72 hours of a scheduled shoot.”
